We also support clients with chronic conditions such as substance use disorder,asthma,diabetes, heart conditions, Being overweight, as evidenced by having a Body Mass Index (BMI) over 25,hospice and palliative care, dementia support, care for people living with Alzheimer’s disease, medication reminders.

Services would be provided to elderly and disabled clients who need more extensive care than they themselves, family members, or friends can provide. Care can be provided in the client’s permanent or temporary residence and is not limited to private homes, hospitals, and assisted living facilities. The provider will assist clients who require four-hour home visits, and up to twenty-four hours of care and live-ins.
