Who We Serve

We support clients with chronic conditions such as substance use disorder, asthma, diabetes, heart conditions, being overweight (as evidenced by having a Body Mass Index (BMI) over 25), hospice and palliative care, dementia support, care for people living with Alzheimer’s disease, and medication reminders.

We pride ourselves on providing:

  • Comprehensive care management
  • Care coordination and health promotion
  • Comprehensive transitional care from inpatient to other settings, including appropriate follow-up
  • Individual and family support, which includes authorized representatives
  • Referral to community and social support services, if relevant
  • The use of health information technology to link services, as feasible and appropriate.
  • Diet management and so much more

Services would be provided to elderly and disabled clients who need more extensive care than they themselves, family members, or friends can provide. Care can be provided in the client’s permanent or temporary residence and is not limited to private homes, hospitals, and assisted living facilities. The provider will assist clients who require four-hour home visits, and up to twenty-four hours of care and live-ins.
